Event Form

We’re excited that you want to host your upcoming event at Tab Church! Please complete the form on this page. Alternately, you may download a printable copy here and submit it to the church office in person, or via email to Sarah Hall shall@tabchurch.org as soon as possible.

We offer a wide variety of event and meeting space large and small, from classrooms to banquet hall and gymnasium. All spaces include roundtables, long tables, and chairs.

  • Terms and Conditions: By applying to hold your wedding at Tab Church, you agree to abide by all terms and conditions outlined in the Event Form.
  • Approval: You will be notified whether your request is approved within 3 business days.
  • Fees: Fees vary and will be discussed with you after completion and approval of your form.
